As outlined in Florida Statute 117.05(5)(b), a Florida notary may rely on any one of the following forms of identification, provided the document is current or issued within the past five years and includes a serial or identifying number:
a. Florida driver license or ID card issued by an authorized public agency
b. U.S. passport issued by the Department of State
c. Foreign passport stamped by the U.S. Bureau of Citizenship and Immigration Services
d. Driver license or ID card from another U.S. state, territory, Canada, or Mexico
e. U.S. Armed Forces identification card
f. Veteran Health Identification Card (U.S. Department of Veterans Affairs)
g. Florida Department of Corrections inmate ID (issued on or after Jan 1, 1991)
h. U.S. Bureau of Prisons inmate ID
i. Sworn statement from a law enforcement officer confirming identity when IDs were confiscated at confinement
j. ID card issued by the U.S. Bureau of Citizenship and Immigration Services
